Required Sports/Physical Forms
Annual Physicals and Sports Clearance Information
1) Students must submit an annual health certificate. Athletes may not participate in pre-season training or regular sports season unless they have been cleared with a health assessment or physical by their physician. 2) Interval Health History for Sports Participation form
must be completed by a parent/guardian prior to the start of tryout
sessions or practice at the beginning of each season unless the student received a full medical examination within 30 days of the start
of the season.
- For fall sports approval, please submit the form after July 19, 2011.
- For
winter and spring sports, please submit the Interval Health History
form within 30 days of the start of that season. Winter sports season
begins November 14, 2011.
- Winter Sports Athletes, please see the school nurse for clearance starting Friday, October 14th.*
3)
Athletic Emergency Card - parent and athlete must each complete and
sign this blue card that is mailed to you. Additional copies are
available at the school nurse's office.
All forms/card must be returned by each student to the nurse's office during clearance times before the school year begins.
2011-1012 School Year - as follows:
- Students whose last name begins with A through M: Thursday, August 11, 2011 - 9am to 1pm
- Students whose last name begins with N through Z: Friday, August 12, 2011 - 9am - 1pm

*Any
student needing a physical exam may receive one at Clarkstown South
High School on Thursday, October 27, 2011 at 3:30 pm. Please let our
school nurse, Mrs. Forlini, know so that she can give your name to the
doctor prior to the exam.
